318eea33ae refactor(cli): extract AddEnvironment/RemoveEnvironment functions 
- support upsert in AddEnvironment, no-op RemoveEnvironment if absent
- fallback active env to devnet on remove, add tests
- integrate with sandbox create/destroy, ignore core/plans/
